I experienced the same troubles and as far as I remember the problem
has to do with static and dynamic color maps (and so called visuals)
in X. Even if this sounds paradox, you should switch back to 8 bpp and
try it again. Furthermore, don't run any other applications (such as
Netscape and the like) and try to keep your window manager (fvwm) as
simple as possible (no fancy window bars, colorish backgrounds, etc.)
since all these programs allocate lots of colors on their own. As far
as I remember, the simulator tries to allocate at least 40 private
colors. Only the 8bpp mode offers the appropriate visual to do this
and then it depends if you have still available those 40 colors or
not.

I'm not sure if this gets you rid of your problems but give it a try!
