It is vague, as most ITU-T specs are specific on protocol details that
affect interworking, but details that influence operational aspects are left
"as an exersize to the reader".

Pretty much.  Come up with something good and maybe we'll all add it to the
next version of the standard.

It doesn't have to be, I've seen a couple of vendors tack on some sort of
port Id to help in fault isolation.
