Fore ATM Spans (Lane 0.4) driver for NT 4.0?

Anybody aware of where to get a copy of FORE ATM driver for NT 4.0 (Intel
Platform)?  The card that I am using is Fore PCA200E.  I am particularly
looking for the driver on LANE 0.4 (ie. SPANS).  Thanks & Rgds
